Make a Referral

A referral is required, and we want to make the process as easy as possible. 

Your first step is to download and complete the Referral Form.

All potential participants must complete a Vulnerability Index Service Prioritization Decision Assistance Tool (Vi-SPDAT) to determine eligibility for Long Term Supportive Housing.  Prospective participants must also meet the Minnesota definition of long term homelessness by being homeless for one year or four times in the past three years.
